Ebola Doctors Are Divided on IV Therapy in Africa

Medical experts seeking to stem the Ebola epidemic are sharply divided over whether most patients in West Africa should, or can, be given intravenous hydration, a therapy that is standard in developed countries. Some argue that more aggressive treatment with IV fluids is medically possible and a moral obligation. But others counsel caution, saying that pushing too hard would put overworked doctors and nurses in danger and that the treatment, if given carelessly, could even kill patients.

Partners in Health, using the French initials for Doctors Without Borders, whose staff members have worked on the front lines of Ebola outbreaks for years. “What if the fatality rate isn’t the virulence of disease but the mediocrity of the medical delivery?”
Doctors Without Borders representatives strongly disagreed, saying that Dr. Farmer’s assumptions about Ebola were incorrect, that intensive rehydration would probably not save as many patients as he believes, and that the W.H.O.’s position has not been proved.
The group’s overwhelmed doctors do what they can, officials said, but it is hard to insert needles while wearing three pairs of gloves and foggy goggles. IVs must be monitored, drawing virus-laden blood for tests is dangerous, and patients yank needles out — sometimes in delirium, sometimes just to go to the toilet when no nurse is around.

Researchers have long understood that people are more likely to finish what’s on their plate than to stop eating because they’ve consumed a given amount of food. It’s “the completion compulsion,” a phrase coined in the 1950s by the psychologist Paul S. Siegel. Combine that compulsion with the rising number of restaurant meals Americans eat and the substance of those meals, and you start to understand why we’ve put on so much weight. But there is some good news: As you’ll see below, it’s not so hard to eat bountifully and stay under 2,000 calories. It’s just hard to do so at most restaurants.

Book Review: 'The Big Fat Surprise' by Nina Teicholz

What if the government's crusade against fat fed the spread of obesity by encouraging us to abstain from foods that satiate us efficiently?

It is a commonplace in public-health discussions of obesity to warn that the search for "perfect" or "better" evidence is the enemy of good policy and that we can't afford to wait for all the information we might desire when there is a need to do something now. Yet Ms. Teicholz's book is a lacerating indictment of Big Public Health for repeatedly putting action and policy ahead of good evidence. It would all be comical if the result was not possibly the worst dietary advice in history. And once the advice had been reified by government recommendations and research grants, it became almost impossible to change course. As Ms. Teicholz herself notes, she is not the first to point out that saturated fats have been sinned against by bogus science; and yet, the supermarket aisles are still full of low- and no-fat foods offering empty moral victories.
"The Big Fat Surprise" is more than a book about food and health or even hubris; it is a tragedy for our information age. From the very beginning, we had the statistical means to understand why things did not add up; we had a boatload of Cassandras, a chorus of warnings; but they were ignored, castigated, suppressed. We had our big fat villain, and we still do.

Losing Weight May Require Some Serious Fun

If you are aiming to lose weight by revving up your exercise routine, it may be wise to think of your workouts not as exercise, but as playtime. An unconventional new study suggests that people’s attitudes toward physical activity can influence what they eat afterward and, ultimately, whether they drop pounds.

In aggregate, these three experiments underscore that how we frame physical activity affects how we eat afterward, said Carolina O.C. Werle, an associate professor of marketing at the Grenoble School of Management in France, who led the study. The same exertion, spun as “fun” instead of “exercise,” prompts less gorging on high-calorie foods, she said.

SAN DIEGO — A trip to almost any bookstore or a cruise around the Internet might leave the impression that avoiding cancer is mostly a matter of watching what you eat. One source after another promotes the protective powers of “superfoods,” rich in antioxidants and other phytochemicals, or advises readers to emulate the diets of Chinese peasants or Paleolithic cave dwellers.
But there is a yawning divide between this nutritional folklore and science. During the last two decades the connection between the foods we eat and the cellular anarchy called cancer has been unraveling string by string.
This month at the annual meeting of the American Association for Cancer Research, a mammoth event that drew more than 18,500 researchers and other professionals here, the latest results about diet and cancer were relegated to a single poster session and a few scattered presentations. There were new hints that coffee may lower the risk of some cancers and more about the possible benefits of vitamin D. Beyond that there wasn’t much to say.

Why You Shouldn't Put Your Money Where Your Mouth Is

Talk about dirty money: Scientists are discovering a surprising number of microbes living on cash.
In the first comprehensive study of the DNA on dollar bills, researchers at New York University's Dirty Money Project found that currency is a medium of exchange for hundreds of different kinds of bacteria as bank notes pass from hand to hand.
By analyzing genetic material on $1 bills, the NYU researchers identified 3,000 types of bacteria in all—many times more than in previous studies that examined samples under a microscope. Even so, they could identify only about 20% of the non-human DNA they found because so many microorganisms haven't yet been cataloged in genetic data banks.
Easily the most abundant species they found is one that causes acne. Others were linked to gastric ulcers, pneumonia, food poisoning and staph infections, the scientists said. Some carried genes responsible for antibiotic resistance.
"It was quite amazing to us," said Jane Carlton, director of genome sequencing at NYU's Center for Genomics and Systems Biology where the university-funded work was performed. "We actually found that microbes grow on money."

Study Questions Fat and Heart Disease Link

The new findings are part of a growing body of research that has challenged the accepted wisdom that saturated fat is inherently bad for you and will continue the debate about what foods are best to eat.

45-year-old Chris Dancy is known as the most connected man in the world. He has between 300 and 700 systems running at any given time, systems that capture real-time data about his life.

How Fat May Hurt the Brain, and How Exercise May Help

Obesity may have harmful effects on the brain, and exercise may counteract many of those negative effects, according to sophisticated new neurological experiments with mice, even when the animals do not lose much weight. While it’s impossible to know if human brains respond in precisely the same way to fat and physical activity, the findings offer one more reason to get out and exercise.

Many thought the brain, though, should be insulated from those harmful effects. It contains no fat cells and sits behind the protective blood-brain barrier that usually blocks the entry of undesirable molecules.
However, recent disquieting studies in animals indicate that obesity weakens that barrier, leaving it leaky and permeable. In obese animals, substances released by fat cells can ooze past the barrier and into the brain.

As they grew rotund and accumulated more fat cells, the researchers found, their blood showed increasingly hefty doses of a substance called interleukin 1 that is created by fat cells and known to cause inflammation.
In these mice, as interleukin 1 migrated to the head, it passed the blood-brain barrier and entered areas such as the hippocampus, a part of the brain critical for learning and memory. There, it essentially gummed up the works, the researchers found when they examined tissue from the animals’ brains, which had high levels of interleukin 1 together with widespread markers of inflammation.

The results suggested that, as the scientists write in the study, “treadmill training normalized hippocampal function,” even in animals born to be fat and that remained heavy.

The Full-Fat Paradox: Whole Milk May Keep Us Lean

I have to admit, I melt at the creaminess of full-fat yogurt.
It's an indulgence that we're told to resist. And I try to abide. (Stealing a bite of my daughter's YoBaby doesn't count, does it?)
The reason we're told to limit dairy fat seems pretty straightforward. The extra calories packed into the fat are bad for our waistlines — that's the assumption.
But what if dairy fat isn't the dietary demon we've been led to believe it is? New research suggests we may want to look anew.
Consider the findings of two recent studies that conclude the consumption of whole-fat dairy is linked to reduced body fat.
In one paper, published by Swedish researchers in the Scandinavian Journal of Primary Health Care, middle-aged men who consumed high-fat milk, butter and cream were significantly less likely to become obese over a period of 12 years compared with men who never or rarely ate high-fat dairy.
Yep, that's right. The butter and whole-milk eaters did better at keeping the pounds off.
"I would say it's counterintuitive," says Greg Miller, executive vice president of the National Dairy Council.

A Grizzly Answer for Obesity

So maybe we’ve been on the wrong track. An alternative approach looks at whether nature, through millions of years of evolutionary experimentation, has already figured out how to appropriately cope with biological threats.

Many fascinating examples exist. A diminutive rodent, the grasshopper mouse, is resistant to the excruciating sting of the bark scorpion. For longevity, nothing can top the naked mole rat, which is naturally resistant to types of pain, cancer and, possibly, Alzheimer’s disease. After weeks of fasting, Burmese pythons are able to enlarge their hearts by as much as 40 percent within two to three days of eating in order to accommodate the increased metabolism — a degree of cardiac hypertrophy that would be a leading predictor of mortality in humans.

Hibernation by bears is an astonishing feat of evolution. After an epic period of late-summer gorging, during which, every day, a bear may consume more than 50,000 calories and gain up to 16 pounds, it will fast for up to seven months. Then it subsists solely on stored fat, without eating, drinking, urinating or defecating.
Bears also shut down their renal function during hibernation, resulting in badly scarred kidneys and high levels of blood toxins that would kill a human. What is truly remarkable is that the bears’ kidney failure is reversible: Upon awakening from hibernation, their kidney function is fully restored with no lasting damage.
